the positioner is a finishing device made of silicone of different hardnesses. It is worn an hour each day or more. You bite into it three times per minute. It makes all your teeth sore, but gives a lazy orthodontist a good result because your impressions are sent to a lab that figures out where the ideal place for each tooth is. Then they makde a model that is used to create the positioner device with. That is, if you can stand wearing it.
The positioner will not preserve work or fix distally rotated lower molars as wearing it the required time cannot compensate for the pushing wrong direction from eating and talking.
